Using the Surgeon Locator
A link to our Surgeon Locator is located in the left-hand sidebar of every page on the site. If you choose to contact a surgeon on the list by phone or e-mail, Smith & Nephew will not have any record of your phone call or e-mail message to the surgeon.
Ordering the resource guide
If you choose to fill out the Order the Resource Guide form, we will require your name, mailing address, city, state and zip/postal code. We use this required contact information in order to deliver the resource guide to you by postal mail. If we use a third party to help us deliver the resource guide, we will share your contact information with that third party for that purpose only. We will not sell or otherwise disclose your contact information to any other party.
You have the option to receive additional printed materials by postal mail from Smith & Nephew to add to the resource guide. We will send those materials to you only if you check the box giving us permission to do so. Optionally, you may also give us your e-mail address. If you give us your e-mail address, you may check a box to receive occasional e-mails from Smith & Nephew about joint therapy and related topics. Even if you supply your e-mail address, we will not send you any e-mail message unless you also check the box giving us permission to do so. If we use a third party to deliver these e-mail messages, we will share your e-mail address with that third party for that purpose only. We will not sell or otherwise disclose your e-mail address to any other party.
